Insurance and Fees
If you would like to use your insurance to help with the cost of counseling, I am a provider for the companies listed below and as a provider, accept the reimbursement rate set by the contract. You are only responsible for a co-pay and/or co-insurance and your deductible, if those apply to your coverage. In 2021, there is a $35.00 administrative fee paid at the first session to cover time spent, supplies and expenses in setting up a new client in my practice. This small fee is not covered by insurance, as insurance only covers the clinical interaction we have in a counseling session.
Fees, co-pays and amounts applied to your deductible are due at the time of service, and you may use a MasterCard or Visa based debit, credit or health savings account card, cash or check.

CIGNA Medical, Behavioral Health and EAP
Please note: EAP (employee assistance plan) service is, by definition and required coding for billing purposes, preventive medicine counseling and/or risk factor reduction intervention. Depending on your presenting issues and diagnosis, coverage for counseling may fall under the mental health/behavioral health portion of your insurance policy.
Self-Pay
If your insurance carrier is not listed above, as an out-of-network provider, I will supply you with a detailed receipt showing your payment so you can forward it to your insurance company for reimbursement. Many companies offer out-of-network benefits, typically 50% or higher, which reduces out of pocket cost. Session fees are $140.00 for a 60 minute session and $115.00 for a 45 minute session. The Fee Agreement outlines other charges. The $35.00 administrative charge mentioned above applies to all new clients. Please contact the customer service number on the back of your insurance card and ask for the forms needed to turn in a receipt for out-of-network benefits. I do not file out-of-network claims.
Other Fees
Cancellations/No Show
This is a difficult topic for most small businesses that operate on a fee-for-service basis. Having mutual respect for time invested in the counseling process is important, and also mutually extending grace when something happens that prevents the appointment from taking place. Emergencies happen: illness, severe weather events, changing work schedules, cars break down. I apply grace for circumstances that are beyond your control, however if there is a pattern of frequent changes/cancellations, then the fee is enforced. After two no-shows (not showing up for your appointment and not calling to say you aren't coming), our professional relationship may be terminated. Referrals will be provided, however there is no guarantee that those therapists will have availability. You can also contact your insurance company to obtain referrals.
If you need to cancel/change an appointment, please do so with at least a 24 hour notice. This allows your reserved appointment time to be offered to others who are seeking services. If you cancel with less than a 24 hour notice the cancellation fee is $60.00. The fee is waived if you reschedule within the same week (and if an appointment is available). If you do not show for a scheduled appointment or cancel the same day, the full session fee will be charged.
